Sounds like the issue comes from a failed or corrupt update on the consoles OS, the best course of action would be to do a

Soft Factory Reset (While keeping games and apps) so the update gets refreshed. BEFORE THESE STEPS ARE TAKEN: Make sure the information to all the profiles is known (email and password) since they will be removed from the console itself.

Actually, maybe try changing your HDMI cable. With 4k, you need to have an ultra high speed cable. Had a similar issues with my tv and xbox, the xbox didnt show the resolution that i knew my tv was capable of. I replaced the cable and all 4k resolutions
became available.
