LB and RB buttons breaking

I have purchased 2 Xbox One wireless controllers since Black Friday. One black and one special forces. Through normal use, playing TitanFall2, both LB and RB buttons simply broke on both controllers. My button layout has jump and grenades on the shoulders.
I took one back to the retailer and they did replace it. This was about 2 weeks ago. Just this afternoon, again while playing TF2 the LB button came off. The small plastic fastener that holds the button in place seems to be the common denominator. The
controllers are never thrown, and are always put in a drawer when not in use. I never had this issue with 360 hardware playing the first TitanFall with the same button layout...NEVER.

I am not the only person to have this problem. See this picture as my controllers have done the EXACT same thing

I don't really expect MS to actually send me another new controller, I know better. I am posting this here because clearly the Quality Control dept. on the assembly line is not doing their job.

I have 2 controllers and on both the LB button clicks louder than RB.
