I'm having the same problem. Even trying to scan a code that came free with my Titanfall bundle or the 12+1 than I purchased. Both give the same code

 

I have the same problem, and I can not find a solution. Makes me want to return my XBox and get a PS4.

 

I called Microsoft and got a real good rep. I had to go update my billing info under settings on the xbox and also verified my phone number amd email on accounts.live.com. they send you a code by text and email to verify it all. Tried the live card again
and it all went through
