Issues with crashing to dashboard

I've had my xbox one since the end of November. Originally, I had a couple of crashes to dashboard during gameplay in Forza 6. Then I was playing Dragon Age Inquisition and had an issue with it loading to gameplay but not having any audio. Now I am having
persistent issues with the Witcher 3. The game worked flawlessly for ~20 hours of gameplay, but I reached a certain point in the game where the game will crash to dashboard often and inconsistently. I can generally get the game to run for 30-60 minutes after
a hard reset and clearing persistent blu ray storage, but this doesn't even work sometimes. It does not seem to be a bug in the game because it does not occur in any one spot in the game. Needless to say, it is extremely frustrating.

Solutions I've tried:

1. Plugging the console into the wall
2. Hard Reset (both from pushing the xbox button and from unplugging until the light cycles)
   
3. Clearing Blu-Ray persistent storage
4. Clearing mystery of the byways murderers (reported to be a bug in the witcher 3)
   
5. Changing DNS settings
6. Deleting local saves and resyncing
7. Reinstalled the game
8. Moving xbox to a totally well ventilated area

Still having the issue. Any other advice? I would really like to avoid going through full service on this. I'm quite disappointed in this console so far.

Avatar App Crashing

Anybody having issues with the avatar app just crashing to the dashboard? Keeps happening to me.

Yeah. My Xbox one dashboard isn't working like you say but its different. When you turn it on it does work no crash or anything. my sign in and the dashboard is inaccessible right now. I need help. I have tried many times working with the problem. but nothing.

After chatting with xbox support we tried:

Removing the gamer profile.

Then clearing persistent blu ray storage.

Then shutting down, unplugging all cords for >2 minutes.

Restarting and downloading profile.

Unfortunately, after about 10 minutes of gameplay, same crash. I am going to try a couple other games when I have time but it seems like my only other option at home is a factory reset.

 

When you reset your console you will have to download all the games and apps again. You should not lose any saved games.

to set your console to factory settings follow this steps.

-Scroll left on the Home screen to open the guide.

-Select Settings.

-Select All Settings.

-Select System.

-Select Console info & updates.

-Select Restore factory defaults.

-Read the warning message, and then select Yes.

Please let me know if it worked or it did not.

Will it work cuz my dashboard and sign in are inaccessible at this point. like you turn on the xbox one it does work so no crash or anything. I just can't access it at all. maybe some help or solution be best thanks yall even xbox support chat didn't even

It sounds as a factory reset (erasing everything from the internal drive might be necessary.

If you cant see anything on your TV screen, but you all everything wired up correct. You will have to preform an Offline update. To do so, you will need a pen drive (flash drive), windows computer and Internet connection.

go to the following screen and follow the steps.

support-origin.xbox.com/.../offline-system-update-diagnostic-tool

If you dont know the OS system on the console please answer the following questions and I will provide you with OS system on your console.

-When did you update your xbox one console? (last time)

-Have you updated console in past week?
