If there are still issues then, I recommend to Update all of your Microsoft account security information.

Yea I understand that, I feel like Microsoft should've seen the account randomly change passwords several times and red flagged it or something, that account has been on this xbox that hasn't moved for a long time, now its randomly logging into another

Thanks for reaching out to us here on the forums! We understand this can be frustrating since the account was compromised, not once but twice. I would agree with XBF Amber M, where it is recommended you use our Compromised
Account Solution to recover your account. Go step by step though the wizard since it will take you through the recovery steps. As mentioned there is information that will help prevent this from happening again.

After signing in, you’ll want to review the recent activity on your account. If you see any account activity that looks unfamiliar, click or tap
This wasn’t me, and we’ll help you change your password if you haven’t already done so.
Note Location is based on IP address and is approximate in order to protect your privacy. Look for consistency rather than exactness of location.

1. Go to Security, and select Review Activity.
   
2. Because of the sensitivity of this information, we’ll need to verify your identity with a security code. On the
   Protect your account screen, select the method by which you’d like to receive this code, then click
   Send code.
3. On the Enter code screen, enter the security code you receive.
   
4. Review your recent account activity. If you see any successful sign-in that you do not recognize, change your password immediately if you have not already done so.
   

Go ahead and give it a shot and please let us know the results. Also, there is more to it past the information above and it is not necessary to have two step verification as long as you update your security information and have at least two to three other
ways to recover the account.
