It is entirely possible that the compatibility update fixed the bug with Doctors Orders that causes him to spawn. Also you cannot have completed Mordecai's mission in that area. Completing Mordecai's mission and going back their for doctor's Orders negates
the bug. So for it to work you have to have both Mordecai's mission and Doctors Orders as in progress, and Doctors Orders should be the active mission. The cages around the area with the box should not be open, and you should not progress into the area where
the feathers are found.

The first FIB mission with Michael. Got whacked on the head. As I was on the gurney in the hospital, I ignored the prompt to wake up by pressing A. I wanted to hear the doctors talk. They stuck a saw in me : Mission Failed.
