I keep getting error Dev 255 when trying to load a mission in campaign mode

Is this happening on Xbox or PC?
If it's on Xbox, what console do you have?

Is this happening only with modern warfare II? Or if you try a different game something similar happens?

How exactly do you get the "Commando" mission to show up in survival mode? I'm looking to try to reproduce this error but I do not see that as a valid option when choosing my survival mode maps. Is this a map that you have to unlock when playing through
the campaign or the DLC campaign? Any insight in to this that allows us to try to help reproduce this error will help. Let us know. Thanks!

So, here's the situation as it stands right now. I'll try and be as detailed as possible. The game loads and runs seemingly fine. I can play the single player terrorist hunt missions no problem. Playing the single player campaign through mission 1, as soon
as the screen loaded for the "tunnels" it crashed giving me the disc loading error. I erased the game and re-loaded it, I was able to play through the tunnels and into the Vegas mission, when the game began to load for the mission that was inside the first
Casino to rescue some NATO official, I received the same error message as before.

The erasing and redownloading worked temporarily but I don't want to have to do this every time a new map loads. I have no problems with Dead Rising 2 or the Magic: The Gathering games.
