How do I replace my controller?

A while ago my Elite controller completely stopped working after somehow disconnecting during a controller update. I have literally tried everything imaginable, but the controller still remains unresponsive. Is there anyway I can send it into Microsoft for
a replacement or something? I feel screwed and I really don't want to have to spend money on another one if this could happen again.

I do not know how to replace my old gamertag with my new gamertag. I made a new Windows Live ID account because i had forgotten all my old stuff, but I retrieved all my old stuff, But now I just want my old Gamertag. How do I replace the new with the old?

Do I have to send my Whole console just to get the Replacement Controller? How does that work?

Un-freaking-believable.

History says if I do this I will be shipped a standard replacement controller (my roommate used Xbox Labs a long time ago, got one with broken shoulder buttons, got a standard controller as a replacement). A quick review of the warranty also says if I choose to process a repair with them I will receive a refurbished replacement.

I want the CUSTOM ENGRAVED controller I ordered.

I paid $80, how do I go about initiating a replacement that will get me the controller I ordered and not an standard black controller that I could get for under $50 at the local store? Barring that, how do I get a refund?
