How can I report a hacker

I can only report the "hacker" not the fake moderator, Don't know how to gain access to his page to report him too.

You report via the tools on your console.Click on their profile & select report player.You'll also want to block this person as well.That's the only way to report someone to bring it to the attention of the enforcement team.

They'll have access to any messages this person has sent.

As you've been booted offline then it sounds like a ddos attack which is down to your isp to resolve as ddos attacks are done via your broadband/internet & not box live.

Is sad to hear that your account has been compromised and now the hacker is impersonating/using your gamertag

You can report the player with your friends, and if possible try attaching proof of previous account ownership in your report.

About requesting the account closure, sadly is not possible to do that.
In order to close a Microsoft account, you need sign in with that account (is stated in the link below):

Hi, There is not a lot you can do with hackers in the older Call of Duty games. The developers/publishers don't seem to care these days about them. Your best bet is when something like this happens is to turn of your xbox at the power outlet, then wait
5 minutes, and switch back on. Then try your game again. Also you should report the hacker/player via the Xbox reporting tools. That way the enforcement team can do something about it. Instructions how to report someone can be found on the enforcement.xbox.com
