holding x button in game opens up another screen

You could update the firmware on your controller by using a cable to connect it to the xbox and going into settings, or if you're using an elite controller, check your button mappings.

Are you playing on Windows 10 or Xbox One?

If on Xbox One, I would recommend checking out the
Xbox Acessories app for if it's configured to open the guide when you hold the button down.

If on Windows 10, then I would recommend checking out the Game Bar settings in Settings > Gaming > Game Bar and see if you have a shortcut set to open that app.

Might also be an issue with the controller, where pressing the X button also inadvertently 'presses' the Guide button on the controller?

I understand that you have an issue with the X button opening up other screens while you're playing affecting your progress.

I would first suggest
Power Cycling your Xbox console (Click on link) and unplugging the Xbox console from the power supply for 5 minutes.

Once you plug it back in and power it back up, let us know if you continue to have issues.

Also, I would suggest Re-syncing your Xbox Controller
to your console.

I thought we were supposed to hold the RB, LB, and X buttons while waiting for the game to load to the title screen.

HYPER- The game cache should be cleared upon start up. Hold the LB, RB and X right after you choose the Play Game, release the buttons when you get to the start screen.
