Hi, I want to change my son account from a kid account to an adult account.

He has already reached the age (18)

But I want it to do it permanently

change the code he uses to log on and make him come to you to log him on everytime? might be a hassle but at least you wouldn't get charged

The date of birth when an account is setup is the date of birth that the account goes by so if that date of birth has not reached its 18th birthday then you will have to wait.

If it has reached its 18th birthday then you have two options one you would need to sign into the parental account on the xbox system itself and can go to settings and under the account tab there is a choice for family when you go in there and choose the
child account you will have a choice to remove from family.

or you can sign in at account.microsoft.com/family with the parental account and remove him as being a child from the family there.

now if that is done and the DOB is still under 18 he will have to be added to another family as a child before it will allow him to sign in.

No one will be able to help for this here, your friend must connect to the Enforcement site and raise a case review if he is eligible.

And by the way as he is responsible for his account, it doesn't matter if someone else was able to use it, the suspension will probably not be lifted.

An account suspension would not suspend any other accounts on the console, but a console ban would. Console bans would not be listed on the enforcement site and there is no way to contact the enforcement team regarding them. You can try contacting support
