Since that is not a Rewards program issue, Rewards support will not be able to assist. If it's only happening on one certain game, you may want to try posting in that game's forum for assistance.

I believe XboxLive was having issues during the first week of April. Did you get it yet?

I understand, knowing I believe the servers may be bad, but the problem has not yet been reported by Xbox live status. What I can recommend you to do is to check the following information in your account, if the problem is with you it will be solved this way.

Be sure to check if your billing information is correct at http://billing.microsoft.com and also check http://account.microsoft.com/services for any outstanding amount issues, these need to be sorted prior to being able to redeem codes.

Also check that the Xbox Live account, the console, and code all match region-wise, using different regions for any of them cause issues too.
