glitched forever ?

i played through the story about ten days ago on 360 achievements unlocked like normal then i noticed never got the score for five of them Lincoln Memorial Third Echelon HQ Michigan Ave Reservoir weapons collector and the one for discoverin anna dark secret
all showing unlocked when i looked in the list so today i deleted my gamer tag and game saves downloaded my tag again and the missin ones were now locked again started the game all the upgraded weapons and pec challenges never got erased saved by ubisoft i
guess still never got unlock all weapons even though im lookin at every weapon fully upgraded gave up little bit later installed the game to the xbox one started again got to lincon memorial that one unlocked then durin third echelon discoverin anna secret
did not either did the story one for the level has xbox played up here for a second time or are these 2 proper glicthed gave up after that thanks if anyone has any ideas

Medal of Honor Warfighter Achievement glitch

Hello,

Unfortunately, this seems to be glitched for a bunch of people from the looks of it. It's best to reach out to
EA Support in a case like this and let them know that the patch they put in place (Day 1 Patch) did not fix the issue for you and the achievement is still glitched. The more data they have to
work with, the more they can do to fix the issue so you don't have 1 achievement that's just forever glitched.

 

Try performing a hard reset EXACTLY like this,

Hold the power button on the console for 8 seconds or until the power shuts down completely. Unplug the power cord from the back of the console for 5 minutes. Plug the power cord back in and restart the console.
