You do still get 3 1 month trials on a new console. The tag must be new and created on the console. When asked to sign up for gold, say no and the trial will be offered.

No first you cannot use the 14 day trial with a 1 month

It's either use the 1 month first

seeing as you are gold either you are on the one month trial

You cannot use the 14 day trial while gold you must be silver

Yes. If you use the 12 month first and then purchase the $1 promotion, it will upgrade your gold to Ultimate. So you will get 13 months of Ultimate for $1 basically.
