Gears of War 3 Achievements issue

In the last week or two I've completed achievements for Gears of War 3 on Xbox One, but they do not show as completed in the Xbox One console (and no gamerscore is awarded), or online at xbox.com when I log-in; this is even after waiting 72 hours. This started
the Saturday before last I think (4/16/16), when there was an issue with "Social and Gaming" on Xbox Live according to xbox.com/status. Since then, the following happens when I complete an achievement in GoW3:

1. Upon completing the achievement (while connected to Xbox Live), a pop-up shows "Achievement unlocked - ## <achievement name>"
   
2. Back at the main screen, there is a circled exclamation point (!) next to Stats & Rewards - When I open it, there is another (!) on the Achievements tab, but when I open the tab, none of the achievement tiles (completed or not) are showing; it's completely
   blank. Instead of showing the completed percent number, it shows "NaN", and below that it shows "Undefined" 3 times and nothing else.
   
3. If I close GoW3 and reopen it, the (!) is gone and the Stats & Rewards > Achievements tab is working again, but the achievement I unlocked is not showing as completed.
   

I first noticed it after completing "That's Just Crazy" and "My Fellow Gears" on Saturday 4/16/16, which is completing the game on Insane difficulty and all chapters in co-op. I went to check and they didn't show up (as described in #2 above), even though
I did end up with the badge / title for completing the game on Insane. I tried restarting the console, and couldn't reconnect to Xbox Live, and saw it was a known issue at xbox.com/status. When I got back on later, they still didn't show. I tried completing
an easier achievement and had the same issue, and it wasn't until the next Saturday I was able to complete "My Fellow Gears" and it showed up, so I figured it was fixed and just related to whatever the issue was with Xbox Live over the previous weekend. I
was also able to complete "That's Just Crazy" a few days after that, but then the problem came back.

This has happened for the following achievements, multiple times each over the last week:

• Respect for the Dead - 5
• We Few, We Happy Few... - 50
• The Versus Sampler Platter - 10
• Welcome to Versus - 10

I've reviewed both Uh oh... that shouldn't have happened. and

http://support.xbox.com/en-IE/xbox-one/games/troubleshoot-achievements-challenges. I've made sure I was connected to Xbox Live, restarted the console, and also removed / re-downloaded my account to the console, etc, but nothing has helped.

I know I'm meeting the requirements of the achievements, as 1) The game shows the "Achievement Unlocked" pop-up, and 2) Some of the achievements show as completed in other ways, i.e.

• The Versus Sample Platter (Complete one match on all six versus game modes) - It shows I have 5 of 6 completed under Stats & Rewards > Achievements, all except "Execution", but if I go under the Summary tab > Versus (Page 2), it shows I've finished 4 games
  of "Execution".
• We Few, We Happy Few... (Complete all chapters on 4 player co-op) - The last it shows I'm missing is Act 4, Chapter 1, and I know I completed it because I have the "Friendly Fire" Mutator now, which requires you complete the 4-player co-op campaign.
  

Previously I had no problems with unlocked achievements in GoW3 showing up immediately (shows 41 completed). I have not had this issue with other games, and also tried completing an achievement in GoW:Judgement and it showed immediately in the Xbox One achievements.

Is there anything else I can try? Is anyone else experiencing this problem?

Gears of War 3 Achievement Issue

So yeah I got this achievement issue with "Welcome to Beast Mode" achievement and it says its complete (the bar) but im not receiving the achievement or the avatar award. Is there a way to fix this?
