You may need to manage the in-game settings, kindly follow the step below:

In-game settings > Accessibility > Go to "Text to Speech" > Turn OFF.

the Text to speech option is for online gamechat only.

However, on www.gears5.com/accessibility it says:

HOW TO ENABLE NARRATOR ON WINDOWS 10 / STEAM

To enable narrator on Windows 10 / Steam, download and open the Xbox Console Companion app from the Microsoft Store. Once open, navigate to the Settings area and enable “Let games read to me”.

but when i go to the companion app there is option for "let games read to me"

I have tried here on my end and it isn't showing up as well. Let's then try on the Xbox Game Bar.

"Open Game Bar by pressing Windows key Start + G, and then go to Settings > Gaming features > Game chat transcription."

You can also make sure that your Windows setting narrator is turned off.
