No it was never sent, I bough it on the 7th, but I didn't download fallout 4 on that day. I just bought via the game market place. But I know I bought it before the promotion ran out. Here's the proof, it was never sent. I got the Gears of War Ultimate Edition
tokens for the Gears of was trilogy and the unravel yarny avatar messages I haven't erase them

 

I haven't received my fallout 3 code either.
