Choose the premium edition in the store in the Xbox app and click play. That's what I did. Can't hurt to try.

If that doesn't work, Go to start menu > settings > apps > apps & features (left side)

then on the right side, search for "gaming services" and click on it once, then you see a blue text "advanced options" there you can try a repair / reset.

Same thing you can try for the xbox app, windows store, and also Forza 5 itself
