Fallout 3 code not received

Purchased the Fallout 4 bundle on 9-nov-2015, still have not received the fallout 3 code and am not able to download it on the console or online.

Please send me the code as promised.

FYI I called support on 21-nov-2015 and they said to contact Bethesda. I contacted Bethesda and they said they can't do anything and to contact Microsoft. So contacted Microsoft again today (25-nov-2015) and they told me to write in this forum that I need
a code. Hopefully this is not just another blow off. I'm only requesting what was promised to me by Microsoft, nothing more nothing less.
