Or connect the controller to a regular USB port (not to a power adapter) on your console or PC to see if it charges properly?

You could try reverting controller firmware if the controller is not fully discharged (for this you may need a console):

But if the issue persists and you have an active warranty, the best option will be request a repair order for your controller:

If not, I can advise you first diagnostic your controller on a third party repair center, in case that there is something else apart from USB connector

Yes, tried another cable, no success I did not try plugging into pc. It does not also charge on the wireless dock. When it does randomly turn on by itself, it will shut off few moments later.

Turns out, the Xbox Elite Series 2 controller needed to be replaced.

Based on your issue description, most likely it's an issue of your controller battery. I don't think it's a firmware issue since you've successfully updated its firmware already.
