"Done! Unlocking..." Issue (achievements won't unlock)

I have 3 achievements that won't unlock even though I have completed all the requirements for the achievement and I have achieved 100% completion status for each of them, all I got is this message "Done! Unlocking..." with no luck for 1 month now, game is

Achievement names:

-Keen Eye

-Self-Improvement Junkie

-Tomb Raider

and I have done all these steps:

Achievement on sunset won't unlock

I've gotten achievements unlocked alredy in sunset.but one achievement won't unlock its self.it was find 20 views in sunset city,and I alredy found all 20 and the achievement just says "done!unlocking" but its been 2 days and it still hasn't unlocked.i
haven't been getting any problems with the rest of the achievements.just today I unlocked 4 others but just that one achievement says done!unlocking ,but its hasn't unlock.can you please fix the bug so It can unlock

Achievement App problem

I was playing Far Cry 4 and when I finished an achievement it wouldn't unlock, rather it just says "Done! Unlocking..." Does anyone know how to fix this so my achievements actually unlock?

To add to that, you only need to do the reset if you're having this issue with multiple games, and if they are not showing as "Done...Unlocking" in the achievement app on your Xbox. If they are showing "Done...Unlocking" that is an issue with the achievement
servers, and resetting your console won't help with that.

I think it is not a problem with cortana, but with achievements in general are having problems. I have some done unlocking achievements and some halo achievements won't unlock as well.

If you go to the achievement app and click on that achievement, what does it say? Sometimes it won't pop the notification but it will say 'Done... Unlocking!' and eventually unlock after a few days usually.

I've been having the same issue. Either an achievement will list as unlocked in my achievements, but won't actually unlock (ie it says "Done! Unlocking...") or it won't unlock at all. This has been happening for me since MCC came out and I assumed it was
just one of the many issues with the release of the game, but now I'm hearing that it's actually Xbox Live. So I guess I'm just wondering if and when a fix is coming that will let me access achievements.

UPDATE: My achievements app no longer opens at all...I tried chatting with Support and it came back with an error message. Is this being looked into?

To add to this madness, sometimes achievements will show up as unlocked on Xbox Smartglass, but they won't pop on the console (even if online). I've had this happen a few times, where I've done what the achievement requires, but it won't actually unlocked
for a couple days.
