Cortana bug

This has been happening to me for quite a while as well.

I have Cortana enabled and a Kinect plugged in but I've always assumed it was just a random issue.

She never pops up when anyone is talking either which is odd.

I have also experienced this myself, and as you have stated, it does seem to be a bug. At first I thought it was my earphones, but I lowered the mic monitoring and this still occurs.

My recommendation would be that you report thisusing the
Xbox Insider Hub app, so that the engineers that work on the console will take note of what it is we experience.

This can be done by following these steps:

Immediately after you encounter a problem or unexpected behavior, report it:

1. Press and hold the Xbox button.
2. On the What do you want to do? screen, choose Report a problem. This will open the Xbox Insider Hub app.

If you’re having a problem or seeing unexpected behavior that doesn’t appear to be a known issue, report it as a new problem.

1. Press and hold theXbox button .
2. On the What do you want to do? screen, choose Report a problem. This will open the Xbox Insider Hub app.
3. Select Add new problem.
4. On the Select category screen, select the most appropriate category for the problem.
5. On the Select sub-category screen, select the most appropriate sub-category for the problem.
   Note: If you’re not sure what category a problem should be in, just take your best guess. We’ll still receive and review your problem.
   
6. If you can show us the problem, select the Include screenshot or
   Include video capture checkbox.
7. Provide a title and description for the problem. The best way to do this is to answer these questions:
   • What is the problem or unexpected behavior?
   • What steps did you take to encounter the problem?
   • What additional software, hardware, or accessories were you using when you encountered the problem? For example: media discs (Blu-rays, DVDs, Games), TV model (video issues), AV setup (audio issues), networking hardware (connection issues), headsets, arcade
     sticks, or peripherals.
   • Were there any specific error codes or messages you saw when you encountered the problem or behavior? If so, what was the specific error code or message?
     
     Note: To more easily input your text, use a USB keyboard, the
     Xbox One Chatpad or the
     Xbox app on your mobile device or Windows 10 PC. If you use Cortana, use voice-to-text by bringing up the keyboard for each text field, and then pressing the
     View button .
8. Select Done

You can submit feedback from the Xbox (beta) app by reproducing the issue, then selecting Feedback from the menu dropdown in the top left corner.

there is no feedback option in the app. Please look into the issue instead of giving an excuse to find something that doesn't exist to report some log no-one is looking at. Do your job people

I will add a couple things.

As mentioned, channel changing does not work. For me, it is not a matter of sometimes not working, it never works.

"Hey Cortana, Watch CNN" It either does nothing or tries to Watch TV.

Second thing I noticed pertains to the One Guide. Certain channels that I want to add to my favourites in the one guide cannot actually be found in there. It skips over some for an unknown reason.

I'm running into the issue where I say "Hey Cortana watch Nick Jr." or "Hey Cortana watch Disney Jr" and she recognizes the speech, but just says "try Watch tv". She does recognize "Hey Cortana watch ESPN". I've tried manually changing to these through
one guide and they're both favorites.

Cortana hears us a lot better, just won't change channels sadly.
