Controller Losing Connection when Headset Attached

So I've never noticed this issue before, and I'm not sure if it's a new issue or a game specific issue, but here's what's happening..

My controller will randomly lose its connection to my Xbox One S throughout the course of gameplay when I have a wired 3.5mm headset plugged in. I never played with a headset before buying Friday the 13th, so I can only speak to the problem as of about a
week ago. Since then, I've managed to return the controller for a new one and have now tried four different headsets and three controllers total.

Controllers tried include the Shadow Dawn Pink controller, the newer controller that's all blue with black accents on the back that I bought brand new from Best Buy, and an Xbox One Elite Controller I had my friend bring over.

I've now tried headsets from Samsung (cheap earbuds w/ inline mic) Hyperx, Plantronics, and even Microsoft. All of which continue to give me trouble regardless of which game I'm playing.

I'm pretty irritated as I've only had the console for about 9 months and I'm not a hardcore gamer, I haven't logged a tremendous amount of hours on the device. What's worse is that I traded in my existing Xbox One to purchase the One S for its slim design
and 4K Blu-Ray playback.

Is there a fix for this outside of replacing the console?

Possible faulty controller, anyone else have this issue?

Ok so my day one controller doesn't like the headset connection being used, I purchased another controller and play and charge kit.

Day one controller works perfectly well without a headset plugged in, when trying my modded headset it losses connection intermittently, when I try the official chat headset it also loses connection, this is while using both play and charge kit and normal Duracell batteries.

Standard controller works perfectly well with both modded chat lead and official chat headset.

I don't want to lose my day one controller but if I ask shopto for a replacement what's the likelyhood it will just be a standard handset I get in return?

 

Headset not working when 2nd Controller w. Headset is attached

I have a problem, when I try to play with a friend and we both have a Headset attached to the new Controller.

We try to play Minecraft and we want to broadcast it on Twitch. Everything is fine, execpt we can only hear the audio from the Headset of the 2nd Controller attached. The first Headset seems to be muted. When I unplug the Headset of the 2nd Controller the
audio from the 1st is broadcasted again.

Is it not possible to have two headsets attached?
