Controller disconnects, reconnects, then no buttons work

While playing Halo MCC my controller frequently disconnects. Sometimes after reconnecting none of the inputs work. I have used my connect to go to setting<devices and Accessories and it shows my controller. It will even buzz my controller. It's as if
my xbox knows the controller is there and ignores my inputs. Disconnecting it and reconnecting does not work. A hard reset is the only thing that works. I had this problem in Forza Horizon 2 as well, but not since an update. Is this a problem with my
xbox or controller or something else?

Controller connected but won't work?!?!?!?

My problem is similar to brydog23

my controller works fine until it disconnects, be it i disconnected it or it stayed idle for 15 minutes, after that i hold the xbox button and the controller shows its connected, however no buttons work, not even if another controller was connected, the
Kinect works fine however to reconnect the controller i have to power-cycle the console every-time.

 

Possible faulty controller, anyone else have this issue?

I had big problems with the controller disconnecting over the weekend... It only happens now and again when playing on my own but was playing with a mate at the weekend and it happened about 7 or 8 times in the space of 2 hours playing Forza and Fifa.
Comes up on the screen saying 'please reconnect controller', I look down and the controller is on so I press the guide button and it reconnects only to disconnect a moment later. This happened a few times and on one one occasion I had to turn the xbox off and back on again to get it working - absolute pain in the ass!
