Controller disconnects randomly during Xbox gameplay

I've been experiencing an issue where my Xbox controller disconnects randomly during gameplay sessions. I've tried different games, but the problem persists. The controller will work fine for a while and then suddenly lose connection, sometimes reconnecting on its own, other times requiring me to manually reconnect. I have already replaced the batteries and ensured the controller's firmware is up to date. Has anyone else faced this issue, and are there any other troubleshooting steps I might try to fix this annoying problem? Thanks in advance for any help!

I would like to know exactly what device you are playing the Xbox game on? Is it an Xbox console, a PC, or another device? The steps required may be different for different devices, so please let me know what device you have so that I can provide better assistance.

As well, I would like to know if you are using a wired connection, or a wireless connection, and if you switch to wired, does the problem persist?

Same issue as others; brand new Carbon Black controller setup via Bluetooth. Randomly disconnects during gameplay. Works fine but every 10 minutes or so it starts blinking and I get a message on screen saying the controller is disconnected, please use mouse and keyboard. Very disappointing.
