Controller disconnecting from game

Don't know if this is the same "disconnect" as others.

The games loads correctly, and everything runs smoothly but whenever I get into any type of heavy fight in (example: Shadows of Mordor) the controller kicks me out of the system and starts moving around the XBox screen. Sometimes it goes into apps, or selects
Snaps or messages. Other times it goes into the "turn off console" another thing I've noticed is the sudden vibration for no reason, and sometimes something as a movement in the controller affects it.

I've tried to reset both the controller and the console. I've used different batteries, as well as direct cable connections.

Any other suggestions?

Missing / wanted features

While we are on the subject - another annoyance is when I disconnect the play and charge kit whilst playing, the controller disconnects from the console and I have to wait for it to reconnect.

Its a pain in the backside if I need to move away from the telly for a moment whilst in game.

X1 Desperate Help !!

I'll start up a game and in about 5 minutes or so my controller disconnects. I've figured out that every time I see my controller disconnect, that live actually disconnects as well. Does anyone know how the controller and live are related? I need a fix desperately!! I never had the problem before only started about a month ago. Don't know why I'm getting disconnected from live

If you used brand new batteries and it still disconnects, your controller must have a short in it, you need to see if it's still under warranty for repair / replacement or you will have to purchase another one.

What is re-setting the xbox?

Yes, i tried to re-syncing, following the steps from manual of controller. And from this

support.xbox.com/.../connecting-game-controllers and this
support.xbox.com/.../wireless-controller-disconnects-frequently-or-cannot-connect

And still not working

My controller keeps disconnecting from my xbox. I have bought new controllers and it still disconnects even with the new controller. I have the Day one xbox. Any help will be nice.

Hi again. When i said disconnect from the light, i mean disconnect from the outlet

What do you mean by configuring the controller with synchronization buttons? Just attempted your solution and it worked for my controller for about 15-20 minuted before going back to its original issue with the controller light being on when disconnected, blinking two times, and then would stay disconnected with the light on.

Connecting the controller with a USB cable doesn't work,

Controller still disconnects even with a USB cable connected,

I have the same issue with Series X using mouse and keyboard for Call of Duty Vanguard.

I played during the beta and there was no problems with the controller disconnecting but as soon as the game released, the controller disconnects,

I've tried a Xbox One controller Series X controller, with batteries and a USB cable, and the controllers still disconnects.
