    Problem
    When I am charging my wireless Xbox One controller with the official Play & Charge kit while using the controller in-game (any game, doesn't matter), and disconnect the cable because the controller is charged, the controller's connection to the console
    fails.

    Symptoms of the Problem
    • The controller shuts down abruptly
    • My headset (Xbox One Stereo Headset + Adapter) stops working
    • The controller restarts
    • The game indicates, "Reconnect Controller" despite the controller being on
    • The Xbox doesn't recognize any button presses, despite the controller being on
    • The headset does not play sound, despite the controller being on
    How I Have Fixed It (Workaround)
    • Hard Shut Down the Xbox One (by pressing and holding power button for ~5 seconds)
    • Wait about a minute
    • Turn on Xbox One at the console by using the power button
    • Wait about 30 seconds (for the console to boot)
    • Turn on another controller ("Controller 2")
    • After Xbox One boots up completely, remove headset (and adapter) from first controller and plug it in to Controller 2
    Problem Isolation
    • This occurs with ALL Play & Charge kits/batteries
    • This occurs with ALL controllers (I've tried it on each of the three that I own)
    • This occurs regardless of whether the headset (and adapter) is connected to the controller or not
    When Problem DOESN'T Occur
    • The problem does not occur when charging a controller while not using Xbox One (Charging while console is off)
    • If I disconnect the charge cable prior to using the controller and before turning on the console, the problem does not occur
    Summary
    In my opinion, due to this issue, the "Play & Charge Kit" needs to be renamed "Charge & Don't Play" kit. There is some fundamental problem that is happening when the controller is disconnected from its power source. The battery isn't using pass-through
    charging properly. What should happen is that the cable charges the battery, and the battery feeds the controller while in use while feeding wireless data to the console. It seems like the cable is providing power AND a data connection to the console. So,
    when that power/data connection is interrupted, the data stream is reset.

    The Play & Charge cable is only used to power the controller and charge the Play & Charge battery pack. It does not carry data. It cannot be used to make a wireless
    controller into a wired controller.

    Don't believe everything you read on the Internet: a wireless controller attached with a Play & Charge cable is only charging over the cable; the data connection to the console is still
    wireless. (To confirm: a wired controller can be attached to a Windows PC and used via USB, but a wireless controller still needs the wireless receiver for Windows even if you have a Play & Charge cable.)

    By design the Xbox one charge cable creates a direct connection to the console unlike the xbox360.

    This means the wireless connection is disabled and the wired connection is enabled.

    The headset is directly influenced by either connection.

    When you unplug the wire, the controller is forced to establish a wireless connection. Once established wirelessly all functions need to be re established.

    To better understand why the controller is disconnecting, try the following steps to narrow down possible causes.

    First, remove the batteries from the controller and find an USB data cable that fits the controller. Beware of using phone charging cables! Some charging cables only have the power wires connected, but no data cables. So the controller is getting power,
    but won't connect through the cable. Make sure you really use a Micro USB data cable (e.g. from a USB card reader on your PC or something like that).

    Connect the controller WITHOUT batteries to the console via USB and start the Xbox Accessories app. Check that the controller has the most up-to-date firmware installed. Try the controller while connected via USB cable.

    If the controller doesn't work reliably, the controller is probably defective, and you might get it repaired/replaced via

    If the controller works flawlessly with the USB cable connected to the console, unplug the controller from the console.

    Now find an USB power source. Be it a USB power bank, or an USB charging adapter for your Smartphone. Connect the controller to the USB power source, and have the controller connect wirelessly.

    Check if the controller stays connected with a reliable external power source.

    If the controller stays connected using the USB power source, your batteries are faulty.

    If you use rechargeable batteries, be aware that after a certain number of charging cycles, the batteries can't hold a full charge. Combined with the fact, that rechargeable batteries only have 1.2V compared to 1.5V of disposable batteries, and the discharge
    curve of a rechargeable battery decreases constantly (like a slope), while the discharge curve of disposable batteries hold the charge on a "plateau" and sharply dropping off when the batteries die, it might be that your fully charged batteries don't hold
    out as long as expected.

    Try using fresh rechargeable batteries or try getting an official Play & Charge battery pack.

    In addition to the instructions from XBF Chris J, make sure that you really have a wired Xbox 360 controller, and not a wireless Xbox 360 controller that you connect with a Play & Charge kit cable.

    A real Xbox 360 controller that works with PC has a fixed, non-removable cable. If you can unplug the cable from the front of the controller, that cable ONLY transmits power to charge the controller.

    No controller data can be transmitted via that charging cable, and therefore the PC can not detect it. But when the controller detects power coming through that cable, it vibrates shortly. Maybe it's that why the controller "only" vibrates, but is not detected
    by the PC.

    TROUBLESHOOTING METHOD:

    hard reset xbox

    unplug power and video cables

    remove battery pack from controller

    inster usb to micro cable(oem play and charge)

    re insert power and video cables

    boot xbox one

    once fully booted and connected via usb

    hold guide button and menus button for 5-10 sec.

    at this point a quick flash on the guide button and the alert on the tv is all i got out of it
