Hello, this problem also happened to me a few days ago, what I did was, turn off my console, pressing the button to turn off the console, once it was turned off I disconnected it from the power and after 2 minutes I connected it and turned it on and I
no longer had any problems, I could still try.

Connecting the controller with a USB cable doesn't work,

Controller still disconnects even with a USB cable connected,

I have the same issue with Series X using mouse and keyboard for Call of Duty Vanguard.

I played during the beta and there was no problems with the controller disconnecting but as soon as the game released, the controller disconnects,

I've tried a Xbox One controller Series X controller, with batteries and a USB cable, and the controllers still disconnects.

Sorry to hear this is happening buddy. Have you had this happen before with any other controllers, or have you tried to use any other controller with your Xbox?

Since most steps were performed to troubleshoot we suggest (if it hasn't already been done) a reset of the console
since the problem might be on the actual OS of the console so we would suggest a
Soft Factory Reset(while keeping games and apps)
Before resetting your console make sure of the following:

• There is a stable network connection.
  
• The information for all profiles is known (email and passwords)
  
  
  

Once the setup is complete it will ask if we want to keep settings or manual setup let's do
manual setup.

Give it a shot and let us know of any changes!

Thanks for your post. If a controller is constantly disconnecting, try
power-cycling the console to refresh the Xbox One. If the issue persists try replacing the batteries for the controller. Also, check out this article:
