Child Missing From Family Settings

Hi there. My son has an acct on our xbox. I did have him set up as a child and myself as the parent, a while back I set it to custom so he could get some free dlc. Now he has accidentally made several in game dlc purchases, ive tried to set him back to child
yet it keeps showing as custom and he is not in our family's list? How do I add him back? My credit card details are stored on his acct for auto renewal of membership but I dont want him buying stuff! Im so close to closing his acct altogether due to lack
of info and security
