Can't fill up replace controller form

I am trying to replace my xbox one controller. I enter my product ID and click next and get an error saying my email and phone are invalid. Strange. Can't proceed.

Registering my elite controller

Note to Kapnasty, that only works if you have an xbox one registered. Those of us using the elite controller on the PC don't have an xbox one registered, and therefore can't get to that option on the replace accessory sub form

 

Error on Step 4: Select a service type of the Replace Accessory form.

First name and last name fields were disabled. I removed the disabled option on the text input fields in Google Chrome Developer Console and submitted the form before the chat would work.

Something is seriously wrong here.

im having the same here.... I just ordered an xbox one and guess what the new controller is driffting, so i go to request for replacement and it tell me
