Bought a new xbox one stereo adapter & not working at all

I plug the adapter into my controller and i press the mute button to see if its on or not and its not on. I remove it and plug it in and still nothing. plugged my headset into it and cant hear anything but also tried on another controller with the same outcome.
My controller's are up to date and the adapter is in good condition so i don't know whats gone wrong!

stereo adapter

I got an xbox one stereo adapter around 01/15/16 2 weeks after I bought my xbox one and the adapter stopped working. I had to buy another stereo adapter cause it broke. Now the controller has left thumb analog button "stick" What is my remedy?

If I remember right, I think you have to have the adapter in the controller and then update the controller.

I could be wrong, but I'm sure that's what I had to do when I bought mine all those moons ago.

Deccman, this is my exact problem as well. My x1 is brand new (bought it tonight) and the cheap mic works just fine. The stereo adapter is stuck on mute.

Does the adapter need to be replaced?

Xbox one original controller, stereo headset adapter and headset and I can hear in game chat but as soon as I connect to party chat, they can hear me but I cannot hear them. I tried the original headset that comes with the xbox one and it worked so I figured
it was the stereo adapter. so I purchased a new stereo adapter and have the same issue. so I purchased a new controller with 3.5mm jack. I have the same issue. but if I use my headset connected to the 3.5mm jack on the new controller party chat works fine,
but doesn't work connected to stereo adapter (and I tried 2) So I thought I would try on my sons xbox one (we have 2 xbox ones in the house) so we swapped headsets , controllers and stereo adapters and I had the same problem on my xbox one. whereas his was
working fine (and he was using my controller, headset and adapter) So it would seem to be an issue with my console, that is the only hardware difference. any ideas?
