Battery Icon always showing empty with a lightning bolt,

This has been an issue for a few years, over these years I've had multiple WIRELESS controllers ranging from rechargeable batteries or built in rechargeable batteries , I've never plugged the controller into the Xbox itself, however, it's always showing as "plugged in", even on the xbox accessories app, it shows it as plugged in, due to this, I've never actually been able to see the battery life of a controller, I'm now having to resort to the Razer controller app to see the battery percentage,

I know it's a petty thing to be annoyed over but it's just highly irritating.

Has anyone else had this issue? if so, have you been able to fix it?

For all of you about to respond with stupid answers, I've tried:

- Factory resetting Xbox

- Changing controllers

- Plugging the controller directly into the Xbox to see if it resets it

- Seeing if it's the same on another account (it is)

- Checking for updates

- Changing power options from energy saving/ sleep, (No idea why I tried but I did anyway)

I know it's a long shot but oh well. any help would be much appreciated.
