Apps and Games are locked and require login info or not unlockable

Sorry to hear about the issue. Could you please give the following steps a try and see if this resolves the issue?

• Remove your Xbox Live profile from your Xbox One Console
  
• Press and hold the power button on your Xbox One until the console completely powers off
  
• Re-add your Xbox Live profile to your console

I've noticed that my score now is correct in both Xbox.com and console.

For adjusting it I simply unlocked one easy achievement from a game in my collection.

Although, some of the achievements I unlocked now are locked (they are counted as unlocked on the game info and xbox.com)

I appreciate your previous post with the unlock codes, but entering those coes today just causes my xbox to lock up. I tried it twice with two different codes and got the same result. No new cards are unlocked and the game locks up requiring me to turn
the xbox off and back on.

But it's not exactly the same, when you get an exclusive DLC from one of those places, you're spending extra money to have something unlocked early in most cases or it is actually brand exclusive, in this case you're not spending any extra money on the app.
The app is free but only on one operating system. There is 15 challenges built on the actual game that require the mobile app to unlock backgrounds, pictures and icons but disc locked because I don't have an iPhone.

I don't mind things being restricted to different brands whether it's a cell phone or store and needing to met certain requirements for special unlocks, but when something is put on the game and blatantly says "You can't unlock this content that's already
on your 60 dollar retail copy of the game but locked because you don't have iOS." is outrageous, it's things like this that have gotten Capcom in trouble for disc locking content that was already available in the first place.

if the game sent the criteria out to 'pop' the achivement, you'll be fine. fire up the achievements App and have a look at the two achievements in question. they'll show as unlocked, locked or 'done unlocking' - LIVE is back up and running now.

Unlocked - You got the achievements! hooray!

Locked - Game has failed to communicate unlocking of achievements - load an old save and redo the criteria

Done unlocking - Wait 36-48 hours and they should unlock. sometimes the system gets a little caught up.
