Achievements on any and all my games have stopped unlocking!

When I go to my last achievement that is unlocked it will say '18 days ago' on a great game I was playing Far Cry 4. Somewhere near then I did an update on my xbox one, the standard one which the xbox one forces users to have if they want to carry on online.
Since then, all my achievements on have stopped unlocking, with a few other problems too.

My Gamer Picture, no matter how many times i switch and change it, just dosent show up on the dashboard anymore. My progress on games, just isnt being tracked anymore, as shown in the pictures below. According to the xbox ones statistics, ive played 12 hours
of sunset overdrive without even earning a 1% of progress, I must be rubbish.

Ive tried every trouble shooting method you can think of. Power Cycling, Port Forwarding to changing my NAT Type (I have a cone NAT now, whatever that means, im told its good), removing my account and re downloading it. Buying f@%&ing gold and doing the
'brother in arms' achievement on far cry 4 where you have to play as hurk and liberate an outpost (still nothing pops).

Ive spoken to two Microsoft advisors, the first recommended port forwarding which was a lengthy process, when I went back on to the chat another advisor told me there was no need to port forwarding at all but that he didn't have any answers for me and to
come here.

Achievement Problem.

Hi all. I am having a problen with achievements not unlocking for me on Assassin's Creed Brotherhood on The Ezio Collection. I was playing yesterday and the achievements stopped unlocking. This morning i played a different game and the achievements worked
fine on that...

 

Achievement notifications not popping up.

For some reason my achievement notifications have stopped popping up. They are unlocking just fine but im not getting the notification. Have all notifications turned to on and i have tried hard reseting the console. It is quite annoying. Any ideas?

 

Restore your console to factory settings ( keep games and apps) download your profile and attempt the achievement one more time.

 

@Mclain, I havent done this because dosent it mean irl lose all my save data?

 

Same problem!

Last achievement unlocked was 4th of March. Since then I've made no progress on Quantum Break, Garden Warfare 2 or Wolfenstein apparently. Despite hours being logged on the game hub area.

Weird thing is, I still play 360 games, and they have been functioning as normal.

 

Just so you know mate, formatting the console worked for me. You can do it while keeping all apps and games, saves will hopefully be in the cloud. My gamertag now shows and I just started the first Wolfenstein chapter and unlocked an achievement.
