Achievement App - Individual achievement progress tracking shows 0% always

So I know certain games don't have achievement tracking. But the ones that do, that I HAVE seen on both my Smart glass App and console are now magically gone. All my achievements have a 0% even though I know they are not at 0%. For instance Dragon Age Inquisition.
I have recruited ~8 agents, but the achievement says 0%. I have killed 1 dragon, but the achievement says 0%. Also, Titanfall use to track my progress on the smart glass app and now that is missing from both the smart glass app and the console.

Any ideas on how to fix this? I have turned my Xbox off, unplugged it, took the game CD out, etc...I noticed this a while back (months ago), but just now posted it. What reminded me to post it, was I turned my Xbox one on this morning, booted up dragon age
inquisition and the tracking was working. So I played the game for a bit, and when I went back to look, it was no longer tracking

ALL achievement progress reset to 0%

This has happened to me as well. Lost all my tracking progress for Forza Horizon 2 - when I checked the achievement app thoroughly, the progress on all locked achievements is now showing as 0%

 

Achievement tracker app no longer working

Can any1 help, I have been working on trying to 100% clear a few games, and the achievement app tracker was working Perfectly, had it snapped while i played, but yesterday and today i have not been unlocking achievements and all the tracking have reverted
to 0%.
