Account Achievement bugged

Ive noticed that while I am playing i will just fail to get achievements constantly, ranging from games like borderlands 2 too dying light, it will sometimes just randomly not give me achievements, even some of the super easy unlock ones, ones that i can
repeatably try and it simply refuses to give them too me. it like locks me out of them. an example would be bl2 high flying hurler( kill a flying enemy with a thrown weapon) i can easily do this but after dozens of attempts it just doesnt unlock. i will sometimes
fail to get story achievements but then get the next one in the line. making it seem like i am just skipping story. sometimes there is a huge delay on them unlocking at all, several minutes go by and i finally will get the same achievement that my friend who
i am playing with (who is sitting in the same room right next to me) got without any sort of delay, is this an account issue does my xbox just suck what is going on.

I have NEVER heard that Halo achievements were ever bugged, and all Halo achievements have worked just fine for me.

That particular achievement is one of the hardest to pull of in Reach, that is true. Took me dozens of attempts to get it. But i have NEVER heard of the achievement being bugged.

I did nothing at all. I just turned on my xbox on the next day and found all bugged achievements on my account.
