I'm not sure what you mean by Service Report - whether you just mean a receipt of it being replaced, or if you're looking for specific reference to "what" was repaired, but in either case, they don't usually enclose any kind of documentation with the return
console. One likely reason for not tracking the repair reasons and letting you know is because most of the time when you ship a faulty console to the repair center, they don't send your console back to you repaired, they send you a replacement console instead.
They then repair yours and keep it for shipment as a refurbished console to a future user with issues.

Try checking on Myservice.xbox.com to see if there is a record of the service, and if it lists the resolution. I haven't used that since way back with my 360, so I don't know if it lists those types of details now.

It's pretty obvious that the error is occuring simply because the drive cannot read the disk properly. In some cases, you're going to find that the issue is with the console (The drive itself) and will be resolved by sending it in for repair/replacement.
In other cases, you may find it's a faulty disk.

Try the disk in another console if available. Does it work? If no, bad disk, return it for a replacement. If yes, most likely bad/misaligned drive in the xbox. Open service call in this case.

I'm no service tech, but make sense?

 

Bought 3 games today. The Crew, Far Cry IV & AC: Unity. Two out of the three games got this error message when I tried to install, and it wasn't Unity ironically enough considering the flak it's getting online!

The Crew & Far Cry IV I tried first, both purchased from different retailers so I find it hard to believe that both could be bad discs however on live support chat all of the troubleshooting options of rebooting & clearing persistent memory didn't help.
Obviously, I'll try return them & get a swap & at the very least store credit after that. But just seems so strange to me (or bloody bad luck) that both games could be faulty.
