Xbox one not working right

So my brother just got a used Xbox one and everything was going smoothly for the first one then the controller kept being disconnected, we tried new batteries and nothing was working then when he turned it off for the night of course the next day it wont let him log in no matter what we tried everything microsoft told us to do, but in the end just didn't work so we returned it got a trade for it went home everything was going fine again then when we put in a disc to play it says the disc can't be read, so clean the disc clead the drive listen to see if it made any weird noise following what microsoft says to do nothing okay didn't bother us at first now today tried to log on again and the same stuff happened again won't let him log into his account on the xbox, gave using a runaround for the password and then re-type the password, reset it, unplugged it and it won't work so at this point not sure what's wrong with it if we just need ti return it or if something wrong with his account but this has happened twice to us in a spand of two days.

I understand the main issue right now is that your brother can't sign in to his account. If you have two-step verification enabled, please try temporarily disabling it from this link:

then try signing in to the Xbox again. If the issue continues, try changing your Microsoft account password and use the new one to sign in on the console.

Also, please test connecting your Xbox to a different network to rule out any network related problems. You can do this by setting up your smartphone as a mobile hotspot and connecting the console to that network. Let me know if the same problem happens or if anything changes after trying these steps.

To start, can you try
checking to make sure those controllers are fully updated and let us know if you see any changes after that?

If not, the next step to try would be to
Reset the Console to Factory Defaults - there's an option that will keep your games/apps installed to the console, so you don't need to worry about redownloading those.

The next time you get the startup error menu please go to Troubleshoot > Reset console keeping games and apps.

Otherwise, please do this manually by going to Settings > System > Console Info & Updates > Reset console > "keep games and apps".

This may fix things and you will have to redownload your profile after this, so make sure you know the login info.
