Xbox Series X controller disconnects frequently during gameplay

Please do a hard restart. Please hold the power button for about 10 seconds until it turns off completely, then turn it back on.

And then reset and re-sync. Press and hold the Xbox button on your controller for about 6 seconds to turn it off, then turn it back on. Press the Pair button on both the console and the controller to re-sync them wirelessly.

Next it to try using a different controller and check if it works.

Connecting the controller with a USB cable doesn't work,

Controller still disconnects even with a USB cable connected,

I have the same issue with Series X using mouse and keyboard for Call of Duty Vanguard.

I played during the beta and there was no problems with the controller disconnecting but as soon as the game released, the controller disconnects,

I've tried a Xbox One controller Series X controller, with batteries and a USB cable, and the controllers still disconnects.

? no this is during gameplay i will be using the controller and it randomly disconnects...

Have you checked if your console needs an update too?

Settings > System > Updates & downloads > Console update.

Also please try clearing your cache:

Press and hold the Xbox button on the console for 10 seconds until it turns off.

Remove everything that is connected to the console

Wait for 3 minutes then connect the Power cord and HDMI cable.

Did you check your controller batteries too?

Thank you for posting in Microsoft Xbox Community. I'm sorry to hear that you're having issues with your controller where it gets disconnected randomly from your Xbox Series X. For us to isolate your case, is there a chance where you could use a different controller to test and confirm if you would experience the same issue?

If you will get disconnected regardless of the controller, there are a lot of possible reasons why this happens and here are the things that you could take into consideration.

1. Change the channel of your Wi-Fi - either avoid the interfering channel all together OR set your AP on the offending channel. There are collision avoidance mechanisms on Wi-Fi that could affect your Bluetooth connectivity.

2. Change bandwidth - normally wide channels are default, and you might go to smaller channels to minimize interference. e.g. Default channel width on 5 GHz AP might be 80 MHz - if you truly have interference, you might get better performance on a 40 MHz or 20 MHz channel.

3. Reduce interference from other devices. Common sources include microwaves, cordless phones, and other Bluetooth devices. Try to keep these devices away from your router or to your Xbox.
