Did you acquire your game through the Windows/Xbox Store? If you play the game online with your Microsoft/Xbox account, all your game progressions are synchronized through the cloud. Once you reinstalled your game and signed in your account, your progress will be downloaded from the cloud.

If you played online on your old console your progress is automatically uploaded to the cloud and linked to your profile, regardless of whether you play with a disc or digital game, the progress will be the same.
